Most are due simply to neglect-- walkways in disrepair with gaps that look down onto a sheer drop of hundreds of feet, for example, or fallen debris blocking the most obvious exit. Ways around these must be found, frequently by pushing blocks, pulling levers, felling pillars, and most often, by making large jumps.
